County treasurer submits:

PA-662 – 2025 Lottery and Gaming and First Dollar Credit Report
- Report the number of lottery credit and first dollar credit claims issued, and total amount paid for each municipality

Due date – March 2, 2026
- County Data File Instructions
- Follow DOR's provided file layout to create your data file
- Use a .TXT file format
- Reconcile the file with Form PA-662
- Submit your file – attach a completed .TXT data file in Form PA-662, Section B before submitting the form

Note: DOR must receive this file before we can process any lottery and gaming credit late claim applications

LC-664 – 2025-26 Manufactured/Mobile Home Lottery and Gaming Credit Report
- Report the number of lottery credit claims your municipality issued and total amount your municipality paid
only for manufactured and mobile homes with a monthly municipal permit fee collected
