The Wisconsin Department of Revenue (DOR) posted the following 2021 Lottery and Gaming Credit forms on our website.

Form LC-100 – 2021 Wisconsin Lottery and Gaming Credit Application

• This application is for property owners to affirm they qualify for the Lottery and Gaming Credit as of January 1, 2021 and is filed with the County Treasurer

Form LC-110 – 2021 Wisconsin Buyer Lottery and Gaming Credit Application

• This application is for property owners that purchased a home during 2020 to affirm they qualify for the Lottery and Gaming Credit because the previous owner used the property as their primary residence on January 1, 2021. It is filed with the County Treasurer.

Form LC-220 – 2021 Manufactured/Mobile Home Municipal Permit - Wisconsin Lottery and Gaming Credit Application:

• This application is for owners of a manufactured/mobile home to affirm they qualify for the Lottery and Gaming Credit as of January 1, 2021 and is filed with the Municipal Treasurer

Form LC-400 – Wisconsin Lottery and Gaming Credit Removal Request

• This form is for property owners to remove the Lottery and Gaming Credit once the property no longer qualifies and is filed with the County Treasurer
