Lottery and Gaming Credit Homeowner Questionnaire If you received a Lottery and Gaming Credit Parcel Audit Notice, complete the Form LC-667 Lottery Credit Eligibility Questionnaire to assist in determining eligibility for your parcel. You will need your Notice Reference Number from your Lottery and Gaming Credit Parcel Audit Notice to complete the questionnaire.
