To: Municipal Clerks and Treasurers, County Clerks and Treasurers

The Wisconsin Department of Revenue made the following forms available in My DOR Government Account. These forms must be electronically filed (e-filed) and are available on our Government Online Filing web page.

County Forms

County Treasurer must e-file both:

• Form PA-662: 2019 Lottery and Gaming and First Dollar Credit Report
  • Report the number of Lottery Credit and First Dollar Credit claims issued and total amount paid for each municipality
  • Due date: March 2, 2020


• Form PC-501: 2019 Municipal Treasurer's Settlement
  • Review the form instructions
  • Counties must electronically file using e-file form or file transfer
  • Due date: March 16, 2020

Municipal Forms

Municipal Treasurer e-file:

• Form LC-664: 2019-20 Manufactured/Mobile Home Lottery and Gaming Credit Report
  • Report the number of Lottery Credit claims your municipality issued and total amount your municipality paid only for manufactured and mobile homes with a monthly municipal permit fee collected
  • Due date: March 2, 2020
