The Wisconsin Department of Revenue (DOR) posted the lottery and gaming credit late claim applications on our website.

The forms listed below are now available on our Lottery and Gaming Credit Forms web page.

• Form LC-300: 2018 Late Lottery and Gaming Credit Application
• Form LC-310: 2018 Late Buyer Lottery and Gaming Credit Application
• Form LC-320: 2019 Late Manufactured/Mobile Home Municipal Fee for Lottery and Gaming Credit Application
• Note: Property owners must submit a complete application, with the appropriate documentation, to DOR by October 1, 2019
