Tips for Common Claim Scenarios

  1. I found property with my former name (e.g., maiden name). How do I claim?

    • Select the Reported Owner – Individual relationship type
    • Enter your current name on the Claimant information tab
    • Attach a document with your previous name, if possible

  2. I found property with my name and another name. How do I file a claim?

    • Select the Reported Owner – Individual relationship type.
    • If the property is in the names of a married couple who filed a joint return in Wisconsin in the past year, the funds may be paid in full to the claiming spouse.
    • If the other owner is still living, you may only be paid your portion of the value of the unclaimed property.
    • The other owner should file their own claim for their portion, unless the other owner is your current spouse, and you filed a joint return in the past year.
    • If the other owner is deceased, provide their date of birth, date of death, and submit a copy their social security number.

  3. I found property with my former address. How do I file a claim?

    • Enter your current address on the Claimant information tab
    • Upload and attach documentation with your name and that previous address, if possible

  4. I found property with my name but no address. How do I prove this property belongs to me?

    • Submit any documentation showing a connection to the holder/company that reported the property
    • Examples: Copies of uncashed money orders and cashier’s checks

  5. I found property in the name of a trust, such as the John Doe Irrevocable Trust or the Jane & John Doe Family Trust. How do I claim?

    • Select the Heir or Personal Representative relationship type
    • Attach a copy of the trust
    • If the trust is still active/open, submit the FEIN or tax identification number of the trust
    • If the trust is closed/inactive, provide a statement saying it is closed and provide a copy of the trust

  6. I found property in a relative or friend’s name. The court appointed me as their Personal Representative. How do I claim?

    • Select the Heir or Personal Representative relationship type
    • If the estate is open, enter the FEIN of the estate and submit a copy of the Domiciliary Letter from the court
    • If the Estate is being probated outside of Wisconsin, provide the documentation form the court granting you the authority to act on behalf of the Estate.

  7. Where can I find a deceased relative's social security number?

    The death certificate will provide the social security number. You can order a death certificate through the WI Department of Human Services by calling (608) 266-1865 or visit their website at: https://www.dhs.wisconsin.gov/vitalrecords/index.htm.


  8. I am helping someone with a claim and need assistance. What should I do?

    • You and the claimant call our office together to discuss the claim OR
    • Complete Form A-222, Power of Attorney and submit with the claim so unclaimed property staff can discuss the claim with you
