website.
Form Information

Due date – November 2, 2026
- Must e-file in
MyDORGov
- Provide municipalities in your district with the same apportionment values and referenda information reported to DOR on this form
- These values auto-fill on the municipality's 2026 Statement of Taxes
Referenda Information (in Section A of form)
You must enter the referendum Information if
both the following apply:
- Your technical college approved a referendum for a temporary tax levy increase after December 31, 2014
- Referendum increases your technical college's current year (2026) tax levy
