The Wisconsin Department of Revenue, State and Local Finance Division would like to remind you of today's filing deadline for the Video Service Provider Report (Form SL-311).
General Information

Due date – August 17, 2026
- Municipal clerk must e-file through
MyDORGov
- Only municipalities that impose a video service provider fee under state law (sec.
66.0420(7), Wis. Stats.) must file

Do not e-file Form SL-311 if your municipality does not currently receive video service provider aid
Form Information
- DOR uses the submitted form to confirm whether your municipality qualifies for the 2027 video service provider aid (2019 WI Act 9)
- Each municipality that imposes a video service provider fee must report if it imposed a fee in 2025
Note: If a municipality no longer imposes the video service provider fee since it received video service provider aid and the mandated decrease of the video service provider fee decreased the fee you were imposing to 0%, you must still submit this form to receive its aid payment.
