The Wisconsin Department of Revenue, State and Local Finance Division would like to remind you to file the Video Service Provider Report (Form SL-311) if you have not already done so.

- Municipal clerk must e-file through
MyDORGov
- Only municipalities that impose a video service provider fee must file (sec.
66.0420(7), Wis. Stats.)

Do not e-file Form SL-311 if your municipality does not currently receive video service provider aid
Form Information
- The reported certification is used to confirm qualification of the 2026 video service provider aid for Wisconsin municipalities under state law (2019 WI Act 9)
- Each municipality that imposes a video service provider fee must report if it imposed a fee in 2025
Note: If a municipality no longer imposes the video service provider fee since it received video service provider aid and the mandated decrease of the video service provider fee decreased the fee the municipality was imposing to 0%, the municipality must still submit this form to receive its aid payment
