- Municipal clerk must e-file through
MyDORGov
- Only municipalities that impose a video service provider fee under state law (sec.
66.0420(7), Wis. Stats.) must file

Do not e-file Form SL-311 if your municipality does not impose a video service provider fee
Form Information
- DOR uses the submitted form to confirm whether your municipality qualifies for the 2026 video service provider aid (2019 WI Act 9)
- Each municipality that imposes a video service provider fee must report whether it imposed a fee in 2025
Note: If your municipality no longer imposes the video service provider fee since it received video service provider aid and the mandated decrease of the video service provider fee decreased the fee you were imposing to 0%, you still must submit this form to receive your aid payment.
