Special District Boundary Changes Form Now Available

The Wisconsin Department of Revenue (DOR), State and Local Finance Division posted the Special District Boundary Changes (Form PC-650) on our website.

General Information

  • Due date – March 31, 2025
  • Must e-file through MyDORGov
  • This information is used during the annual certification of your district's equalized value

Form Information

  • All special districts must file the Special District Boundary Change form
  • Indicate if your district has annexed or detached territory from January 1-December 31, 2024
  • If your district has annexed or detached parcels, you must include:
    • Recorded ordinance or authorizing document annexing or detaching territory filed with the county register of deeds
    • List of parcels annexed or detached from the territory
    • Map reflecting the new boundaries of the district
