The Wisconsin Department of Revenue (DOR), State and Local Finance Division will soon post Form PC-250 – Annexation School & Special District Boundary Inquiry on our website. This new form will replace the paper versions of the monthly "Annexation – School/Special District Inquiry" letters.

Form Information

• If your municipality was the annexor in an annexation you will receive an email notification from DOR that you are required to file this form
• Due date: End of current month
• Your municipality must e-file through MyDORGov
