• Due date – today, March 3, 2025
• Municipal treasurer must e-file through MyDORGov
• LC-664 – located on our website
• Important – if this form is not filed timely, payment distributions will be delayed

Filing Detailing

• This form is only for manufactured and mobile homes where the monthly municipal permit fee is collected in 2025
• Report the number of lottery credit claims your municipality issued and total amount your municipality paid only ​for manufactured and mobile homes with a monthly municipal permit fee collected

Note: If you do not have manufactured/mobile homes receiving monthly municipal permit fees, you are not required to file this form
