The Wisconsin Department of Revenue, State and Local Finance Division (DOR) would like to remind you that Form PA-662: 2022 Lottery and Gaming and First Dollar Credit Report is due today, March 1, 2024.

Form Information

  • County treasurer must e-file through MyDORGov
  • PA-662 – located on our website
  • Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality
  • If this form is not filed timely, payment distributions will be delayed

County Data Files - Updated Instructions

Send your lottery and gaming credit post-certification county data files to DOR through MyDORGov by today, March 1 2024.

This file must:

  • Be complete and follow the file layout provided by DOR
  • Reconcile with Form PA-662

Submit file to DOR:

  • Lottery and gaming credit post-certification county data files must be submitted in a .TXT file format
  • Attach a completed .TXT file to your Form PA-662 (located in MyDORGov) before submitting the form
  • Note: DOR must receive this file before we can process any lottery and gaming credit late claim applications
