New Alcohol Beverage License Forms are Available for Use

The Wisconsin Department of Revenue (DOR) initiated a review and update of several alcohol beverage license application forms that municipalities use. With the assistance of municipal clerks and the retail alcohol beverage industry, they are now complete and ready for use! For this license year (July 1, 2023 – June 30, 2024) clerks may use either the new license forms or the old license forms. For next license year (July 1, 2024 – June 30, 2025) the department will require use of the new forms.

Major changes to the forms include:

  • Updated format with a more streamlined look
  • Companion instructions for each form with citations to statutory guidelines, administrative rules, and DOR-provided resources
  • Plain language questions

New Alcohol Beverage Retail Licenses are located on the department's website. The forms are fillable PDFs.
