The Wisconsin Department of Revenue (DOR) would like to remind you the Form LC-664: 2019-2020 Manufactured/Mobile Home Lottery and Gaming Credit Report is due today, March 2, 2020.

Form Information

• Must be electronically filed in My DOR Government Account and is available on our website
• Report the number of lottery credit claims your municipality issued and total amount your municipality paid only for manufactured and mobile homes with a monthly municipal permit fee collected
• If this form is not filed timely, payment distributions will be delayed
