MyDORGov
- Provide municipalities in your county with the same apportionment values and referenda information reported to DOR on this form
- These values auto-fill on the municipality's 2025 Statement of Taxes and Tax Increment Worksheet
Referenda Information (Section B of form)
You must enter the referendum information if
both the following apply:
- Your county approved a referendum for a temporary tax levy increase after December 31, 2014
- The referendum increases your county's current year (2025) tax levy
