The Wisconsin Department of Revenue (DOR) would like to remind you that Form PA-662: 2021 Lottery and Gaming and First Dollar Credit Report is due today, March 1, 2022.

Form Information

  • You must e-file through MyDORGov
  • PA-662 – located on our website
  • Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality
  • If this form is not filed timely, payment distributions will be delayed

County Data Files

Send your Lottery and Gaming Credit post-certification county data files to DOR by today, March 1, 2022.

This file must:

  • Be complete and follow the file layout provided by DOR
  • Reconcile with Form PA-662

Submit file to DOR:

  • Email completed file to lgs@wisconsin.gov
  • Note: DOR must receive this file before we can process any lottery and gaming credit late claim applications
