Order your library's Wisconsin income tax forms by using our library forms order system. Sign up for email updates from the Wisconsin Department of Revenue.

• Select the "Governments" tab
• Select "Libraries"
• In the "Order Forms" box, select "Forms Order System"

Step 1: Enter the following information on the "Library" screen.

• Phone Number
• Contact Name
• Library Name
• Shipping Address
• City State Zip

Step 2: Select "NEXT." Enter the number of forms needed in the box in front of each form type.

Step 3: Select "NEXT" and review your order.

• If your order is not correct, select "Previous" to return to the previous screen and correct the information.
• If your order is correct and you want to print a copy of your order, right-click and select the "Print" option.
• Select "Place Order" to complete your order.
