All Forms WT-6 must be OCR encoded per ABA specifications. The scan line contains 27 characters including:

  • Depository bank drawer number for withholding tax (3)
  • Space (1)
  • Wisconsin tax account number (15)
  • Check digit (1) for WTN
  • Space (1)
  • Filing frequency (1)
    • 1 = Semi Monthly 1-15 of month
    • 2 = Semi Monthly 16 - EOM
    • 3 = Monthly
    • 4 = Quarterly
  • Period ending month (2)
  • Period ending year (2)
  • Check digit (1) for Filing Frequency Code and Period Covered

Other OCR Requirements

  • Use OCR "A" font.
  • Place the OCR print (scan line) ½ inch from the bottom of the coupon. The last character in the scan line must be ½ inch from the right hand edge of the coupon.

OCR Check Digit Calculation

  1. Multiply each digit by a weight pattern of 2, 1, 2, 1, 2.
  2. If the product is two digits, add the two digits together.
  3. Add all digits together.
  4. Take the sum and subtract from the next multiple of ten.
  5. The result is the check digit.

Example: Filing Frequency 10818

  1. 1 X 2 = 2
    0 X 1 = 0
    8 X 2 = 16*
    1 X 1 = 1
    8 X 2 = 16**
  2.   *Since the product of 8 X 2 is two digits, add the two digits : 1 + 6 = 7
    **Since the product of 7 X 2 is two digits, add the two digits : 1 + 6 = 7
  3. 2 + 0 + 7 + 1 + 7 = 17
  4. 20 - 17 = 3
  5. 3 is the check digit for the filing frequency 10818

Ink

  • Black ink

Form WT-6 size

  • 8 1/2 inch width
  • 3 2/3 inch height

Paper

  • White, highly opaque, with a flat finish.
  • Weight 24 pounds/500 sheets.
  • Smoothness between 65 and 200 Sheffield units on both sides when measured with a Sheffield Tester.

Data Requirements

  • Wisconsin Tax Account Number (WTN) 15-digit number with check digit.
  • Check digit - All Forms WT-6 must contain a check digit after the Wisconsin Tax Account Number (WTN) and in the OCR scan line at the bottom of the Form WT-6.
  • Calendar year the form covers
  • Federal employer identification number
  • Filing frequency
  • Period covered by form
  • Preparer's name typed, date signed, preparer's title, and preparer's telephone number
  • Amount of Wisconsin tax withheld
  • Taxpayer's legal name and complete address
  • Scanline

Form WT-6 Substitute Approval

The Department of Revenue requires approval of Forms WT-6 that are not issued by the department prior to use. Developers/payroll providers must submit 20 to 30 data-filled test forms clearly marked in red as TEST FORMS to the department for approval.

The test packet should include:

  • 5 unique data-filled forms for quarterly accounts
  • 5 unique data-filled forms for monthly accounts
  • 5 unique data-filled forms for semi monthly accounts for the 1st -15th of the month
  • 5 unique data-filled forms for semi monthly accounts for the 16th to the end of the month

Substitute forms developers will be notified via email when testing is completed, generally within fifteen business days of receipt.
