The Wisconsin Department of Revenue (DOR) posted the 2017 State and County Apportionment Form (PC-400) on our
website.
Form Information
- You must electronically file (e-file) this form

Due date - November 15, 2017
- You must provide municipalities in your county with the same apportionment values and referenda information reported to DOR on this form
- These values will auto-fill on the municipalities' 2017 Statement of Taxes
Reminder - Report Referenda Information
You must complete the referendum section if:
- Your county approved a referendum for a temporary tax levy increase after December 31, 2014
- The referendum increases your county's current year (2017) tax levy
Getting Started (to e-file)
New platform!
- Form PC-400 now requires email authorization
- Same reporting requirements with a new look and feel
- Works with any browser
Accessing the form:
- The first time you access the form on our
website, you are asked to enter your email address
- Go to the email message with the subject "Login authorization link" and click the provided link
- Select the Form PC-400 and begin filing
- Note: You must start and finish email authorization using the same browser (i.e., Chrome, Edge)
Filing the form:
- Complete the form – you must enter an amount in each required field
- You can save your work periodically
- After submitting, print and/or save a copy for your records
