TO: Municipal Clerks and Treasurers

Please disregard if you've already filed your Form LC-664

This is a reminder that Form LC-664: Manufactured/Mobile Home Lottery and Gaming Credit Report was due to the Wisconsin Department of Revenue (DOR) on March 1, 2017.

If your municipality does not submit Form LC-664 to DOR, your Lottery and Gaming Credit payment will be delayed.

To electronically file (e-file):

1. Go to the DOR website and select Form LC-664
2. Enter the number of Lottery and Gaming Credit claims and the total dollar amount being applied to the 2017 manufactured and mobile home monthly municipal permit fees in your municipality. Report the amount for the entire year.
3. Submit form and save a copy for your records (including your confirmation number)
   Note: To file an amended form, access the form on our website again
