The Wisconsin Department of Revenue would like to remind you that Form PA-662: 2017 Lottery and Gaming and First Dollar Credit Report is due tomorrow, Thursday, March 1, 2018.

Form Information

• Must be electronically filed and is available on our website
• Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality
• If this form is not filed timely, payment distributions will be delayed
