The Wisconsin Department of Revenue would like to remind you that Form LC-664: 2017-18 Manufactured/Mobile Home Lottery and Gaming Credit Report is due tomorrow, Thursday, March 1, 2018.

Form Information

• Must be electronically filed and is available on our website
• Report the number of lottery credit claims your municipality issued and total amount your municipality paid only for manufactured and mobile homes with a monthly municipal permit fee collected
• If this form is not filed timely, payment distributions will be delayed
