This is a reminder that the following items are due to the Wisconsin Department of Revenue (DOR) on March 1, 2017.

Form PA-662: Lottery and Gaming and First Dollar Credit Report

• Available on our County Forms page
• Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality
• Note: To file an amended form, access the form on our website

Lottery and Gaming Credit Post-Certification County Data File

This file must:

• Be complete and follow the file layout provided by DOR
• Reconcile with Form PA-662

Submit file to DOR:

• Email completed file to lgs@wisconsin.gov
• Note: DOR must receive this file before we can process any Lottery and Gaming Credit late claim applications
