Comments Requested by March 13: Municipal Alcohol Beverage License Forms Update

The Wisconsin Department of Revenue (DOR) initiated a review and update of several alcohol beverage license application forms that municipalities use, and would appreciate your feedback. Please review the DRAFT alcohol beverage license forms on our website and provide comments by March 13, 2023 (COB). We intend to publish the updated, finalized forms on our website in mid-March, shortly after the comment period.

Major changes to the DRAFT forms include:

  • Updated format with a more streamlined look
  • Companion instructions for each form with citations to statutory guidelines, administrative rules, and DOR-provided resources
  • Plain language questions
