The Wisconsin Department of Revenue (DOR) posted the 2019 Form PA-632A: Statement of Taxes (SOT) on our website today, November 22, 2019.

Form Information

• Each municipality must electronically file (e-file) the SOT
• Due date: December 16, 2019
• Your taxing jurisdictions' apportionment data will auto-fill. You cannot e-file until all your taxing jurisdictions submit their apportioned tax levies to DOR.
• If your municipality is required to complete the Tax Increment Worksheet, you must complete it before completing the SOT
