The Wisconsin Department of Revenue (DOR) would like to remind you both forms listed below must be electronically filed (e-filed) by March 1, 2019. If either form is not filed timely, payment distributions will be delayed.

Form PA-662: 2018 Lottery and Gaming and First Dollar Credit Report

• County must e-file on our website
• Report the number of lottery credit and first dollar credit claims issued and total amount paid for each municipality

Form LC-664: 2018-19 Manufactured/Mobile Home Lottery and Gaming Credit Report

• Municipality must e-file on our website
• Report the number of 2019 lottery credit claims your municipality issued and total amount your municipality paid only for manufactured and mobile homes with a monthly municipal permit fee collected
