The Wisconsin Department of Revenue (DOR) posted the following 2019 Lottery and Gaming Credit forms on our website.

Form LC-100 — 2019 Wisconsin Lottery and Gaming Credit Application:

• This application is for property owners to attest they qualify for the Lottery and Gaming Credit as of January 1, 2019 and is filed with the County Treasurer

Form LC-110 — 2019 Wisconsin Buyer Lottery and Gaming Credit Application:

• This application is for property owners that purchased a home during 2019 to attest they qualify for the Lottery and Gaming Credit because the previous owner used the property as their primary residence on January 1, 2019. It is filed with the County Treasurer.

Form LC-220 — 2019 Manufactured/Mobile Home Municipal Permit - Wisconsin Lottery and Gaming Credit Application:

• This application is for owners of a manufactured/mobile home to attest they qualify for the Lottery and Gaming Credit as of January 1, 2019 and is filed with the Municipal Treasurer

Form LC-400 — Wisconsin Lottery and Gaming Credit Removal Request:

• This form is for property owners to remove the Lottery and Gaming Credit once the property no longer qualifies and is filed with the County Treasurer
