Instructions For Completing The "Treasurer's Transmittal Worksheet" Download Receipts
- Click on "Treasurer's Transmittal Worksheet" tab on bottom of the worksheet.
- Click in first cell under "Date Recorded" column in the "Electronic Real Estate Transfer Summary Section".
- Click on "Data" on the tool bar.
- Arrow over "xml" in the drop down list.
- Click on "import' on the drop down list.
- Find the location of the saved xml file, highlight and click import. (Double clicking the file name will also import the file.)
- Enter all PE-500 (paper) fees collected in the 3-mill or 1 mill boxes.
- Click the "Transmittal Fee" tab on bottom of worksheet. All non-shaded boxes require manual input.
- Enter county name, county number and month and year of Reporting Collections in the boxes at the top of the form where shown.
- Enter Beginning and Ending document numbers from all returns being reported, both electronic and paper.
- Enter Beginning and Ending date recorded from all returns being reported, both electronic and paper.
- Enter the number of PE-500 (paper) returns without a fee in the 3 Mill or 1 Mill boxes.
- Enter any adjustments.
- Enter the information for the following fields: "Name and Address of Preparer", “Title", "Signature" "Date prepared" and "Telephone Number (including area code). (A template can be set up with the county name, county number, preparer and treasurer's information.)
- Save file in Excel format
- Submit payment to bank, see bottom of Transmittal Fee page.
- Submit form to Department of Revenue by e-mailing saved Excel file to: utility@revenue.wi.gov
- Optional: Print saved file and mail or fax to Department of Revenue.
