Wisconsin Donation Symbols
Wisconsin requires individual income and corporation tax return forms to include graphic symbols to highlight the donation entry lines. However an exeption for those forms generated by commercial software packages was implemented for tax year 2007 and thereafter in the budget bill as enacted (Act 20). The loon represents the endangered resources donation, the upper-case "G" represents the Packers football stadium donation, the "VETS" represents the veterans trust fund donation, and the ribbon represents the breast cancer research donation. The loon must appear to the left of the donation entry line on Forms 1, 1A, 1NPR, 1X, WI-Z, 4, 4I, 4T, 5, and 5S. The "G" must appear to the left of the donation entry line on Forms 1, 1A, 1NPR, 1X, and WI-Z. The "VETS" symbol must appear to the left of the donation entry line on Forms 1, 1A, 1NPR, 1X, WI-Z, 4, 4I, 4T, 5, and 5S. The ribbon must appear to the left of the donation entry line on Forms 1, 1A, 1NPR, 1X, and WI-Z.
