- Buyer's Claim for Refund -- must use both Forms S-220 and S-220a
- Seller's Claim for Refund -- for refund of tax reported on your ST-12
- Form ST-12 is preferred (check amended form box). Provide an explanation of changes and supporting documents OR
- A letter requesting a refund will be accepted. Provide an explanation of changes and supporting documents.
- The department will acknowledge receipt of your refund request normally within 6 weeks
