Territory Amendment Packet
(for parcels that are being added)

An active Tax Incremental District (TID) can amend boundaries up to four times by adding whole parcels that are contiguous to the existing boundaries. Each territory addition requires a $1,000 fee and there are several important steps that must be followed to comply with state law. The redetermined base is calculated by adding the additional parcels, at the appropriate equalized value, plus the existing base value.

These forms must be completed and submitted to DOR on or before December 31st based on the amendment resolution date. Newly-annexed parcels must be reported on separate forms other than the municipality's set of forms. DOR uses the information on these forms to establish the TID's amended base value and to verify that the proper legal procedures were followed.

One set (of the forms listed below) must be completed for each amended TID. For your convenience, three different file formats have been provided to complete the TID forms.

  1. Regular PDF - blank forms that you need to print and then enter the data.

  2. Fill-in PDF - can enter the data with your computer and then print. The fill-in forms do automatic calculations, but the form with data cannot be saved.

    Fill-in forms option: Please read the instructions first before using this option. This is not available for all the forms. This option will not electronically file your form.

  3. Excel - has all of the forms in one file. The advantages of the Excel format is:
    • You can save the file and work on the forms periodically.
    • The completed file can be e-mailed to the Department of Revenue.
    • The file does calculations automatically and information can be passed from one page to another.
