    Form W-2 -- When preparing the W-2 Forms, please use your 15 digit Wisconsin withholding tax number.

  4. I am using a private delivery service to send my return. What mailing address should I use?

    Private delivery services cannot deliver items to P.O. boxes. If you are sending tax return information to the Department using a delivery service, you must use the Department's street address:
