Requesting Tax Forms
- How can I get Wisconsin tax forms and publications?
You may obtain most Wisconsin tax forms and publications in one of the following ways:
- Download some forms and publications in Adobe PDF format by returning to the
Forms page or the Publications page.
- Between January and April, check your local library. Many libraries will have a supply of Wisconsin individual
income tax forms on hand during the filing season. In addition, libraries generally have a copy of Wisconsin
Package WI-X, which includes copies of Wisconsin tax forms. Most of the forms in Package WI-X may be photocopied.
- Call or visit any of the Department of Revenue offices located throughout the
state.
