How to Register for Electronic Funds Transfer (EFT)
To begin making EFT payments you must first register with the Department. To request EFT registration materials you have the following options:
- Fill in the online form
- Send your name, address, federal employer identification number, or social security number to the EFT Unit, Wisconsin Department of Revenue, P.O. Box 8902, Madison, WI 53708-8902
- Call (608) 264-9918
You will receive a personalized registration form and instructions upon making your registration request. Follow the instructions to complete the registration form. Choose ACH Debit, ACH Credit, or both payment options. When the form is complete, use your personal EFT identification number and password to register online at EFT Registration and Payment System or call the toll-free registration number (1-888-338-9472). After completing registration, mail the signed registration document to the Department.
